The National Department of Health is seeking a service provider for the construction of a storm water management system at Karatara Clinic in the Western Cape. Key requirements include a written price quotation, bill of quantities, proof of relevant experience, company registration documents, valid tax certificates, Central Supplier Database report, SBD 4 and SBD 6. 1, and BBBEE status level contributor certificate or sworn affidavit. Bidders must have completed at least three civil projects of any value and be registered with the CIDB as a grade 2 CE or higher in civil engineering CE class of works.
In the event of a contract being awarded as a result of points claimed, the contractor may be required to furnish documentary proof. If the BBBEE status level of contributor has been claimed or obtained on a fraudulent basis or any of the conditions of contract have not been fulfilled, the purchaser may disqualify the person, recover costs, cancel the contract, and/or recommend restriction from obtaining business from any organ of state.
